Four rules that do not change

Controlled medication follows an evaluation. It never comes before one.

The prescribing policy

One prescriber, one pharmacy

Controlled prescriptions go to one pharmacy you name, and come from one prescriber.

The state record is checked

Before each controlled prescription we check Arizona's prescription monitoring program.

No early refills, none by message

A request for more, or sooner, is reviewed at a visit.

Prescribing policy

What we prescribe, and when

Written once, so nobody has to ask. Rules for controlled medication come from federal and Arizona law and can change; we tell you what applies before your evaluation.

How fees work

The answers to what nearly everyone asks before a first visit.

  • Insurance we work with

Forge Psychiatry is out of network with every insurance plan, so you pay for each visit yourself. We do not bill Medicare or Medicaid. If you want a prescriber who bills your plan, we will gladly give you the names of colleagues.

    • Out of network with insurance plans

    • No Medicare or Medicaid

  • If you pay yourself

You pay at the end of each visit by card, including HSA and FSA cards. Your fee is set by the length of the visit: 75 minutes for the ADHD evaluation, 20 for a follow-up and 40 for a longer follow-up. We send the fee in writing before the evaluation. We ask for 24 hours' notice to change an appointment. There is no contract and no payment in advance.

  • Receipts for reimbursement

Each month we give you an itemized receipt (a superbill) with the dates and kind of visit, to send to your plan, which may reimburse part of out-of-network care. It carries a diagnosis code your insurer can see. Your plan decides whether it pays anything; ask it before you start.

  • An estimate in advance

If you pay without insurance, you have the right to receive a good faith estimate of what your care will cost before you start. We send one in writing before the evaluation. If the final bill is $400 or more above the estimate, you can dispute it. The estimate is not a contract.

  • Checking your benefits

We do not check benefits, because we do not bill your plan. If you want to know whether your plan reimburses, call it before the evaluation and ask about out-of-network mental health visits.

  • What the first visit includes

The ADHD evaluation is the first visit, and there is no separate consultation before it. It runs 75 minutes: a structured interview, your history and rating scales, then a written summary of what we found and what we recommend.

Questions people ask first

Do you take my insurance?

No. Forge Psychiatry is out of network with every plan and does not bill Medicare or Medicaid. You pay for each visit yourself, and we give you a receipt you can send to your plan.

What is the receipt I can send to my plan?

A superbill is an itemized receipt, with the dates and kind of each visit and a diagnosis code, that you send to your plan, which may reimburse part of out-of-network care; ask your plan first.

What is a good faith estimate?

If you pay without insurance, you have the right to a written estimate of your costs before you start. We send one before the evaluation. If the final bill is $400 or more above it, you can dispute the bill. The estimate is not a contract.

How are fees set, and can I use an HSA or FSA card?

The fee is set by the length of the visit, and we send it in writing before the evaluation. You pay at the end of each visit by card, and HSA and FSA cards work. We ask for 24 hours' notice to change an appointment.

What happens at the first visit, and how often will I come?

The first visit is the evaluation: 75 minutes of interview, history and rating scales. Dr. Madrigal then sets the pace of follow-up visits with you. We do not promise a number of visits.

Do you prescribe controlled medication by video?

It depends on the federal and Arizona rules in force on the day of the visit. We do not start a controlled medication at the visit that begins the evaluation, and we tell you before the evaluation whether follow-ups can be by video or must be in the office.
